Community Health Resources is a community-based behavioral health agency providing services to children, adults and families with mental health and substance use issues in Central and Eastern Connecticut. Administers the Young Adult Community Treatment Team (YACTT), which provides rental assistance for scattered site apartments for young adults ages 18-24.
The Young Adult Community Treatment Team (YACTT) offers one year of rental assistance and longer-term support services to young adults ages 18-24 with mental health or substance abuse issues. Participants must be able to live independently and be engaged in education and/or employment. Support services extend beyond 12-month rental assistance and include case management, nursing care and employment specialist support. Most referrals from DCF or DHMAS. If numbers are low, can take front door referrals, however DCF referrals are given priority.
